BACKGROUND:

The assessment of nutritional status in clinical practice must be done with simple, reliable, low cost and easy performance methods. The power of handshake is recognized as a useful tool to evaluate muscle strength, and therefore, it is suggested that can detect malnutrition.

AIM:

To evaluate the nutritional status by subjective global assessment and power of handshake preoperatively in patients going to gastrointestinal surgeries and to compare the diagnosis obtained by subjective global assessment with traditional anthropometric methods and power of handshake.

METHODS:

A cross-sectional study was conducted with patients for surgery in the gastrointestinal tract and related organs. Socioeconomic and anthropometric data, applied to subjective global assessment and checked the power of handshake, were collected. The force was obtained by the average of three measurements of the dominant and non-dominant hand and thus compared with reference values ​​of the population by sex and age, for the classification of nutritional risk.

RESULTS:

The sample consisted of 40 patients, 24-83 years, and most women (52.5%) housewives (37,5%) and diagnosed with cancer (45%). According to subjective global assessment, 37.5% were classified as moderately malnourished; 15% were underweight by BMI measurements; 25% had arm circumference at risk for malnutrition (<percentil 5); 60% reported recent weight loss; and 37.5% low clamping force in power of handshake on non-dominant hand (left).

CONCLUSION:

A significant association was observed for the diagnosis of nutritional subjective assessment with anthropometric methods and strength of the handshake only at the non-dominant limb.

Background:

Early detection of changes in nutritional status is important for a better approach to the surgical patient. There are several nutritional measures in clinical practice, but there is not a complete method for determining the nutritional status, so, health professionals should only choose the best method to use.

Aim:

To evaluate the total lymphocyte count and albumin as predictors of identification of nutritional risk in surgical patients.

Methods:

Prospective longitudinal study was conducted with 69 patients undergoing surgery of the gastrointestinal tract. The assessment of nutritional status was evaluated by objective methods (anthropometry and biochemical tests) and subjective methods (subjective global assessment).

Results:

All parameters used in the nutritional assessment detected a high prevalence of malnutrition, with the exception of BMI which detected only 7.2% (n=5). The albumin (p=0.01), the total lymphocytes count (p=0.02), the percentage of adequacy of skinfolds (p<0.002) and the subjective global assessment (p<0.001) proved to be useful as predictors of risk of postoperative complications, since the smaller the values of albumin and lymphocyte count and higher the score the subjective global assessment were higher risks of surgical complications.

Conclusions:

A high prevalence of malnutrition was found, except for BMI. The use of albumin and total lymphocyte count were good predictor for the risk of postoperative complications and when used with other methods of assessing the nutritional status, such as the subjective global assessment and the percentage of adequacy of skinfolds, can be useful for identification of nutritional risk and postoperative complications.

ABSTRACT - BACKGROUND:

Surgical patients constitute a group of individuals who are commonly underdiagnosed and undertreated, where nutritional impairment can be either a preexisting finding or a result of the hypercatabolic and hypermetabolic state.

OBJECTIVE:

The purpose of this study was to assess the prevalence of malnutrition, according to the GLIM criteria, and its association with clinical and nutritional factors, in individuals admitted to a surgical unit of a general hospital.

METHODS:

A cross-sectional, retrospective study was conducted, involving patients in the preoperative period due to gastrointestinal diseases. Demographic, clinical, and nutritional data were collected from adult and elderly patients admitted to a surgical unit between March and December 2019. Nutritional risk was assessed using the Nutritional Risk Screening tool (NRS-2002). The prevalence of malnutrition was found using the GLIM criteria. Binary logistic regression modeling was performed to determine the association between the diagnosis of malnutrition using the GLIM method and clinical and nutritional variables.

RESULTS:

The majority of the sample presented nutritional risk (50.2%) according to the NRS-2002. The prevalence of malnutrition according to the GLIM criteria was 32.3%, with severe malnutrition being predominant (21.2%) in all age groups. There was an association between malnutrition and nutritional risk detected by the NRS-2002 (OR: 5.791; 95%CI 3.201-10.478). There was a predominance of patients undergoing cancer surgery (64%) and these patients were more likely to be diagnosed with malnutrition (OR: 2.068; 95%CI: 1.161-3.683), after statistical adjustment.

CONCLUSION:

An important prevalence of nutritional risk assessed by the NRS-2002 and of malnutrition assessed by the GLIM method was identified, especially in its severe form. In addition, preoperative patients with nutritional risk, as detected using the NRS-2002 nutritional screening tool, and candidates for oncologic surgery are more likely to be diagnosed as malnourished using the GLIM criteria.

ABSTRACT - BACKGROUND:

Scopinaro-type biliopancreatic diversion (BPD-S) and its variations are the surgeries that offer the best immediate results in weight loss and regain in the late follow-up. It has a high rate of immediate complications and demands control with frequent laboratory tests.

AIMS:

The aim of this study was to analyze the late postoperative complications of 1570 patients operated by biliopancreatic diversion with gastric preservation laparoscopic video with up to 20 years of postoperative follow-up.

METHODS:

In a follow-up period of up to 20 years, the clinical and surgical complications of 1570 patients with grade II or III obesity were evaluated who were operated on from 2001 to 2014 with the same team of surgeons. Clavien Dindo 11 classification was used for analysis and comparison. Laboratory tests and body mass index (BMI) were used in the analysis of late metabolic outcomes.

RESULTS:

On the one hand, complications in 204 patients were recorded (13%), and 143 patients (9.1%) were reoperated. On the other hand, 61 patients (29.9%), who had postoperative complications were clinically treated with good evolution in 9.2 years (95%CI 8.2-10.3), with a median of 9.5 years (95%CI 6.1-12.9). Gastroileal anastomosis ulcers occurred in 44 patients (2.8%). Patients with malnutrition, severe anemia, or chronic diarrhea were operated on with common loop elongation (n=64 - 4%), conversion to gastric diversion (n=29 - 5%), or reversal of surgery (n=10 - 0.6%). One death was registered throughout casuistry (0.06%).

CONCLUSIONS:

Metabolic result of DBP-S was considered excellent in most patients, even referring to changes in the frequency of bowel movements, loose stools, and unpleasant odor. Complications are usually serious and most of the patients require surgical treatment. Therefore, the biliopancreatic diversion of Scopinaro should be reserved for exceptional cases, as there are safer surgical alternatives with less serious side effects.

Background:

Malnutrition is very prevalent in patients with gastric cancer and increases the risk of morbidity and mortality. Adductor pollicis muscle thickness (APMT) appears as an important objective, quick, inexpensive and noninvasive measure to assess the muscle compartment

Aim:

To compare APMT and other nutritional assessment methods and to correlate these methods with postoperative mortality

Methods:

Forty-four patients, 29 men and 15 women, mean age of 63±10.2 and ranging from 34-83 years, who underwent nine (20.5%) partial and 34 (77.3%) total gastrectomies due to stomach cancer (stage II to IIIa) were preoperatively assessed by Patient Generated Subjective Global Assessment (PG-SGA), anthropometry and laboratorial profile

Results:

APMT better predicted death (p<0.001) on both, dominant and non-dominant hand, and well correlated with albumin (p=0.039) and PG-SGA (p=0.007)

Conclusion:

APMT clearly allowed to determine malnutrition and to predict risk of death in patients with gastric cancer.

Background:

Esophageal atresia is congenital anomaly with high mortality. Surgical complications and changes in nutritional status are common problems after surgical correction.

Aim:

o evaluate nutritional status, esophageal stenosis, and respiratory complications among children who had repaired esophageal atresia.

Methods:

Children aged >2 months old with repaired esophageal atresia were included in the current study. Gender, age, weight, and height were recorded for each case. Height for age and weight for age were calculated for each case.

Results:

According to weight for length percentile, 41.02% of the cases were underweight. Esophageal stenosis was seen in 54.76% of the obtained esophagograms.

Conclusion:

Underweight was present in 41.02 of the patients according to weight-for-height percentile.

Background:

Enterocutaneous fistulas represent a connection between the gastrointestinal tract and adjacent tissues. Among them, there is a subdivision - the enteroatmospheric fistulas, in which the origin is the gastrointestinal tract in connection with the external environment through an open wound in the abdomen. Due to the high output in enterocutaneous fistulas, the loss of fluids, electrolytes, minerals and proteins leads to complications such as sepsis, malnutrition and electrolyte derangements. The parenteral nutrition has its secondary risks, and the fistuloclysis, that consist in the infusion of enteral feeding and also the chyme through the distal fistula, represents an alternative to the management of these patients until the definitive surgical approach.

Aim:

To evaluate the current evidence on the fistuloclysis technique, its applicability, advantages and disadvantages for patients with high output fistulas.

Method:

A systematic literature search was conducted in May 2020 with the headings “fistuloclysis”, “chyme reinfusion” and “succus entericus reinfusion”, in the PubMed, Medline and SciELO databases. Results: There were 29 articles selected for the development of this narrative synthesis, from 2003 to 2020, including reviews and case reports.

Conclusion:

Fistuloclysis is a safe method which optimizes the clinical, nutritional, and immunological conditions of patients with enteroatmospheric fistulas, increasing the chances of success of the reconstructive procedure. In cases where the definitive repair is not possible, chances of reducing or even stopping the use of nutrition through the parental route are increased, thus representing a promising modality for the management of most challenging cases.
